Current bank reference letter including established lines of credit.
Supplier recommendation letters (3 required).
Job reference letters (3 required).
Resumes for all owners and key employees.
Work on hand schedule.
Contractor’s questionnaire/application.
Certificate of insurance.
Current pollution policy, if an environmental contractor

Original general agreement of indemnity.
Current personal financial statements for all owners.
Last three fiscal CPA year end and current business financial statements for all companies.
Bid or final bond report form completed.
Copy of contract/specifications.
Job cost breakdown.
History of companies.
